List of additional 3PLPs to be used for physical distribution- include name of 3PLP and physical address from which product is distributed
Copy of home state license (out-of-state applicants)
Officers & Directors list
List of other states where licensure is held
Designated Responsible Party (DRP) Qualification Review Form
Disciplinary action documents (when applicable)

Criminal History Records (Background) Checks

Applicant facilities physically located in Louisiana:
A criminal history records (background) check is required for any individual person(s) who own greater than 10% interest in the applicant company and the designated responsible party (DRP) individual. Louisiana State Police has changed the process for criminal history record checks to an electronic fingerprint system.
